Life Skills - Day Habilitation Services

Goal

To provide individuals a developmental program to assist and support with the development of functional, cognitive, socialization and community integration skills. This program will only be considered when the Individuals needs cannot be met by any other program at Tenco.

Program Activities

Through this program Individuals will be supported in maintaining and/or enhancing their intellectual functioning in a structured environment that will enhance life skills.  Individuals will be assisted with supports provided for behavioral management to increase their socialization and community integration skills.  Activities may consist of swimming, bowling, movies, going out to eat, shopping, visiting local attractions, cooking, computer usage, visual memory exercises, etc.

Unit of Service

Units of service will be billed in one half or full days.  A one half day of service consists of a one to three (1 to 3) hour day and a full day of service consists of more than three (3) hours, during which a service is being provided by Tenco Industries, Inc. directly to the individual.  Funding agreements should reflect half day or full day units without combination of service codes.

Staff to Individual Ratio

Staff to individual ratio, will be a maximum of 1:5.  Program will be limited to 10 individuals.

 

 

 

 

 

 

Admissions Criteria:

  • Applicant is 18 to 55 years of age.
  • Applicant and Team agree that Life Skills program is appropriate.
  • Applicant has a disability.
  • Applicant is able to meet his/her own personal needs.
  • Applicant is ambulatory or mobile non-ambulatory.
  • Applicant has been sponsored for payment of fees, or able to pay his/her own fees.
  • Applicant is not a danger to herself/himself or others.
  • Applicant is unsuccessful and/or unwilling to participate in Pre-voc activities.
  • In the case where the program is full and individual is an appropriate candidate for the Day Habilitation Life Skills Program their name will be placed on a waiting list with the Rehabilitative Services Director.
  • Minimum Attendance required of two (2) full billable days per week. (Four (4) half (½) days can be planned weekly to meet the minimum of two (2) full billable days per week.)  Note:  This minimum requirement does not include contracted school students.
 

Exit Criteria:

  • Individual shows favorable prognosis for growth from other services.
  • Individual becomes a threat to herself/himself or others.
  • Individual’s behaviors and/or cares can no longer be supported in this program.
  • Individual demonstrates basic independent social sensory-motor and communication skills.
  • Individual can sustain work or non-work activities for duration of one (1) hour.
  • Written request to leave Day Habilitation Services.
